Silahkan pemohon layanan untuk mengajukan permohonan Pencetakkan Id Card Tenaga Kependidikan Baru/Hilang/Rusak ke Staf ULT dengan membawa syarat : Surat Pengantar dari Unit/Lembaga/Fakultas atau formulir yang menyatakan bahwa yang bersangkutan benar Pegawai di USU.
- Pemohon Layanan datang untuk mengajukan permohonan Pencetakkan Id Card Pegawai Baru/Hilang/Rusak dengan membawa syarat yang sudah ditentukan
- Staf ULT akan memverifikasi berkas layanan
- Staf ULT akan meneruskan permohonan ke LPSE melalui E-Ticketing/ langsung
- Admin/staf LPSE akan mencetakan ID Card dan meneruskan ke ULT
- Pemohon Layanan menerima output (ID Card baru)
Jangka Waktu Penyelesaian : 5 ( lima ) hari kerja
